MSSQL login error - Cannot generate SSPI context

If you receive a MSSQL connection error 'Cannot generate SSPI context' when using 'Integrated Security=SSPI' in your connection string and you are using Active Directory for your user authentication then the user entry may not be complete or incorrect for MSSQL authentication.
